Coexistence of Hepatitis B Surface Antigen and Hepatitis B Surface Antibody in the Same Serum.

Abstract
During the clinical evaluation of the radioimmunoassay for hepatitis B markers, we had the opportunity to test serum specimens obtained from 92 cases having simultaneous occurrence of hepatitis B surface antigen(HBsAg) and antibody to HBsAg (anti-HBs) in the same serum.
The following results were obtained.
1) The frequency of coexistence was 6.6%(92/1387), male to female ratio was 1.9 1 and the 80% of the coexisted cases were from the 3rd to the 6th decade¢¥ of. age.
2) With the semiquantitation of HBsAg and HBsAb by ratio of cpm in RIA the coexisted cases could be classified in the four groups
15 cases(16.3%) belonged to Group I (low HBsAg & HBsAb), 45 cases(48.9%) Group It (high HBsAg & low HBsAb),
9 cases(9.8%) Group III (low HBsAg & high HBsAb) and 23 cases(25.0%) Group N (high HBsAg & HBsAb).
According to the distribution of HBsAb RIA units the 61 cases(66.3%) were in the range 0-72 RIA units, and the higher RIA units the less frequency.
3) There were no symptoms or signs in 27.2%. The remainder revealed easy fatigability &/or general weakness(50.0%), anorexia, indigestion, nausea &/or vomiting(28.3%) and epigastric pain, RUQ discomfort &/or tenderness(19.6%). The symptoms and signs were more frequent in group II and less frequent in group IQ.
4) In the liver function tests(AST, ALT, total bilirubin, prothrombin time) normal results were frequent, especially in group I.
5) The clinical diagnosis showed chronic hepatitis(54.3%), asymptomatic carrier(22. 8%), acute hepatitis(8.7%), liver cirrhosis(6.5%), fulminant hepatitis(2.2%) and
hepatoma(2.2%). Chronic hepatitis was more frequent in group II & IV and acute hepatits only in group 11 & N.
6) The pattern of HBeAg(+), HBeAb(--) & HBcAb(+) was the most frequent. The
positive rate of HBeAg is 60.4%, HEeAb 18.0% and HBcAb 92.9%.
7) With the semiquantitation of hepatitis B markers the pair of HBsAg & HBeAg
and that of HBsAg & HBcAb showed significant correlation.
